Improvements

• Cleaner error messages on certain edit conflicts. When an edit refers to a claim that no longer exists, or tries to add a second root expression to a premise that already has one, you now get a clear 4xx response describing the conflict instead of a generic "something went wrong" 500. Real edit flows aren't affected — these are edge cases that surfaced after stale references or rare race conditions.
